1. Assumption of Risk
I, the undersigned participant, acknowledge and understand that participation in mixed martial arts (MMA) and fitness activities at FightLAB Fitness involves inherent risks, including but not limited to physical injury, disability, or death. These risks include, but are not limited to, sprains, strains, fractures, cuts, bruises, and other physical injuries that may result from physical exertion, contact with other participants, or use of equipment.
2. Voluntary Participation
I voluntarily choose to participate in the activities offered by FightLAB Fitness, fully aware of the risks involved. I understand that my participation is voluntary and that I may choose to discontinue my participation at any time.
3. Release of Liability
In consideration of being allowed to participate in the activities at FightLAB Fitness, I hereby release, discharge, and hold harmless FightLAB Fitness, its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, damage, or loss that I may suffer as a result of my participation, including but not limited to those caused by the negligence of the released parties.
4. Medical Fitness
I certify that I am in good physical condition and have no medical conditions that would prevent me from safely participating in MMA or fitness activities. I agree to notify FightLAB Fitness immediately if my medical condition changes in a way that could affect my ability to participate.
5. Indemnification
I agree to indemnify and hold harmless FightLAB Fitness, its owners, instructors, employees, and agents from any and all claims, actions, or demands, including legal fees, arising from my participation in the activities at FightLAB Fitness.
6. Photography and Video Release
I consent to the use of any photographs, videos, or recordings taken during my participation in activities at FightLAB Fitness for promotional or marketing purposes. I understand that these materials are the property of FightLAB Fitness and that I will not receive any compensation for their use.
7. Governing LawThis waiver and release of liability shall be governed by and construed in accordance with the laws of the state in which FightLAB Fitness operates. Any disputes arising from this waiver shall be resolved in the courts of that state.
8. Acknowledgment of Understanding
I have read and fully understand this waiver and release of liability. I acknowledge that by signing this document, I am giving up substantial rights, including the right to sue FightLAB Fitness and the released parties for injuries or damages that may occur as a result of my participation. I sign this document voluntarily and without any inducement.
